You can complete your packet and bring it with you to your appointment or we will give you a packet when you arrive to fill out in our office before your appointment.
Insurance information
We are an ‘In-Network’ provider for most Aetna, BCBS, Cigna, HealthChoice, & United Healthcare PPO Plans and a CMS administrated Medicare provider!
We are ‘Out-of-Network’ with most ‘Medicare Advantage’ styled HMO Plans and SoonerCare.
What to wear?
People often ask what they should wear to their chiropractic appointment… Wear something comfy!
Lots of patients like to wear stretchy pants, gym shorts, leggings under a dress, or loose-fitting jeans.

We offer money-saving membership plans for our patients who would like to receive treatment on a regular basis but do not have insurance coverage.
